Two GLP-1 tablets reached the UK in 2026, which changed this question completely. Until then the best weight loss pills UK patients could access produced modest results compared with injections. That gap has narrowed considerably.

Here is what is licensed, what each one actually does, and how to work out which fits you. Every option below is a tablet or capsule. Injections are a separate category.

Those figures come from separate trials with different designs and durations, so they cannot be compared directly. Treat them as a rough sense of scale, not a league table.

Wegovy tablets

Wegovy tablets contain semaglutide, the same active ingredient as the injection, and were authorised by the MHRA on 11 June 2026 as the first GLP-1 tablet licensed for weight management in the UK.

In the OASIS 4 trial, mean reduction over 64 weeks was 13.6% counting everyone, rising to 16.6% among those who adhered fully. The dose climbs through 1.5 mg, 4 mg and 9 mg to a 25 mg maintenance dose, with roughly a month at each step.

The catch is the routine. It must be taken first thing in the morning after fasting for at least eight hours, with no more than a sip of plain water, and nothing else for 30 minutes. Absorption depends on it. We cover the detail in our guide to Wegovy tablets in the UK.

Suits you if you want the strongest oral evidence base and can hold a consistent fasted morning.

Foundayo

Foundayo contains orforglipron and was authorised on 10 August 2026, making the UK the first country in Europe to license it. It is the only pill here also licensed for type 2 diabetes.

In ATTAIN-1, mean reduction at 72 weeks was 11.2% counting everyone and 12.4% among those who adhered. The numbers are lower than the Wegovy tablet, but the trade is significant: orforglipron is a small molecule rather than a peptide, so it has no fasting, food or water restrictions at all and can be taken at any hour.

Suits you if your mornings are unpredictable, you take levothyroxine, you work shifts, or a fasted window is simply unworkable.

Xenical and Orlistat

 

 

Xenical and Orlistat work on a completely different mechanism. Rather than touching appetite, it is a lipase inhibitor that blocks roughly a third of the fat in your meals from being absorbed. It is taken as a 120 mg capsule up to three times daily with meals.

Results are more modest, typically 5 to 10% over a year, and depend heavily on following a lower fat diet. The side effects follow directly from the mechanism: oily stools, urgency and flatulence, all of which worsen with fatty meals. Treatment is reviewed at 12 weeks and discontinued if 5% of body weight has not been lost.

Suits you if GLP-1 medicines are unsuitable for you, or you prefer a treatment with decades of use behind it.

Alli

Alli is orlistat at half strength, 60 mg, and is the only option here available without a prescription. It is a pharmacy medicine, so a pharmacist will still ask some questions before supply.

Expect around 5% over a year. The same dietary requirements and the same gastrointestinal effects apply, slightly reduced.

Mysimba

Mysimba combines naltrexone and bupropion in a prolonged release tablet, acting on the brain's appetite and reward pathways rather than on gut hormones. The dose builds over four weeks to four tablets daily.

Average reduction is around 8% at 56 weeks. It is reviewed at 16 weeks, stopping if 5% has not been lost. It is unsuitable for anyone with uncontrolled high blood pressure, a seizure history, or certain psychiatric conditions, so the eligibility assessment matters more here than with most.

So which are the best weight loss pills UK patients should choose?

On trial data alone, the two GLP-1 tablets lead. But the pill with the biggest number is not automatically the best one for you, and three factors matter more.

Your daily routine. This is the single biggest practical difference. One GLP-1 tablet demands a fasted morning window, the other demands nothing.

Your medical history. Existing conditions and other medicines narrow the list quickly, often before preference comes into it.

What you can sustain. Every one of these works only while taken, and all of them require a reduced-calorie diet and increased activity alongside.

Eligibility and review points

None of the best weight loss pills UK prescribers can offer are available on demand. Eligibility is assessed first.

For the prescription options, the criteria are broadly consistent: a BMI of 30 or above, or 27 to 30 with at least one weight-related condition such as type 2 diabetes, high blood pressure or sleep apnoea. Orlistat uses a slightly different threshold of 28 with a related condition.

Each treatment has a formal review point. If the expected reduction has not happened by then, the medicine is stopped rather than continued indefinitely. None of these are currently funded on the NHS for most patients, so access is through private prescription following a clinical assessment.

The MHRA advises obtaining weight loss medicines only from a registered pharmacy against a valid prescription, and warns against products sold through unregulated websites or social media.

On published trial data, the Wegovy tablet produced the largest average reduction of the licensed pills.

Do the best weight loss pills UK pharmacies supply actually work?

The licensed ones do, within the ranges above, and only alongside diet and activity changes. Unlicensed products sold online are a different matter entirely.

Can I buy weight loss pills over the counter?

Only Alli, which is orlistat at 60 mg. Everything else requires a prescription.

Are pills as effective as injections?

Not on average. Injections remain ahead, though the gap has narrowed since the oral GLP-1s arrived.
