For many of us, a good cup of coffee is what stands between us and a productive morning. Caffeine is the most widely used stimulant in the world — but how it affects you depends heavily on your own biology.
What caffeine does
Caffeine works primarily by blocking adenosine receptors in the brain. Adenosine accumulates through the day and makes you feel sleepy; caffeine occupies its receptors and temporarily prevents that signal getting through. This is why it increases alertness and reduces perceived effort — and why the tiredness can arrive abruptly once it wears off.
The benefits
- Alertness and concentration. The best-established effect, particularly when you are sleep-deprived.
- Physical performance. Caffeine reliably improves endurance and reduces perceived exertion, which is why it is so common in sports nutrition.
- Antioxidants. For many people in the UK, coffee is one of the largest dietary sources of polyphenols.
- Population-level associations. Observational studies link moderate coffee consumption with lower rates of type 2 diabetes, Parkinson's disease and liver disease. These are associations rather than proven cause and effect, and should be read with that caution.
The downsides
- Sleep disruption. Caffeine has a half-life of roughly 5–6 hours, meaning half is still circulating that long after your last cup. An afternoon coffee genuinely can affect sleep quality that night, even if you fall asleep fine.
- Anxiety and palpitations. Particularly in sensitive people or at higher doses.
- Blood pressure. A temporary rise after consumption, more pronounced in people who rarely drink it.
- Dependence and withdrawal. Headaches, irritability and fatigue are common when regular users stop abruptly. Tapering is easier than quitting outright.
- Acid reflux in some people.
Why people respond so differently
Caffeine is broken down in the liver, and a single enzyme handles more than 95% of that work — encoded by the CYP1A2 gene. Common variants determine whether you are a fast or slow metaboliser.
Fast metabolisers clear caffeine quickly, so effects are shorter-lived. Slow metabolisers have caffeine lingering in the bloodstream far longer, which can mean stronger stimulation, more disrupted sleep, and a more prolonged blood pressure response from the same cup.
Some research has suggested that slow metabolisers may not share the cardiovascular benefits associated with coffee at higher intakes. The evidence here is still developing and not yet settled enough to change clinical guidance — but it does help explain why coffee suits some people far better than others.
Other factors matter too: smoking speeds caffeine clearance, while pregnancy, oral contraceptives and some medications slow it considerably.
How much is sensible?
General guidance for healthy adults is up to 400 mg of caffeine a day — roughly four cups of instant coffee, or two to three of filter or espresso-based coffee. Strengths vary widely, so a large high-street coffee can contain far more than you would assume.
Pregnancy is different: UK guidance advises no more than 200 mg a day during pregnancy.
Remember caffeine also comes from tea, energy drinks, cola, chocolate and some cold and flu remedies.
Practical tips
- Stop caffeine at least 6–8 hours before bed — longer if you are sensitive
- Avoid it as a substitute for sleep; it masks tiredness rather than resolving it
- If you get jittery or anxious, reduce the dose rather than switching to something stronger
- Taper gradually if you want to cut down, to avoid withdrawal headaches
