Exercise is one of the most effective tools available for preventing and lowering high blood pressure — and unlike most interventions, it is free, has no side effects, and improves half a dozen other things at the same time. Here is how blood pressure works, why lifestyle affects it so strongly, and what actually moves the numbers.
What blood pressure means
When your heart beats, blood is pushed out into the arteries. The force of that blood pressing against the artery walls is your blood pressure. It is written as two numbers:
- Systolic (the higher number) — pressure as the heart contracts
- Diastolic (the lower number) — pressure as the heart relaxes between beats
A healthy adult reading generally falls between 90/60 mmHg and 120/80 mmHg. In the UK, hypertension is diagnosed at 140/90 mmHg or above in clinic, or 135/85 mmHg or above on home readings.
Why high blood pressure matters
Sustained hypertension forces the heart to work harder, thickening and stiffening the heart muscle over time. It simultaneously damages the artery lining, accelerating the plaque build-up that narrows vessels. The result is a substantially raised risk of heart attack, stroke, heart failure, kidney disease and vascular dementia.
Crucially, it produces almost no symptoms until damage is done — which is why it is called the silent killer and why measuring it matters so much.
How exercise lowers it
Your heart is a muscle, and like any muscle it responds to training. A stronger heart pumps more blood with each beat, so it needs fewer beats and less force to circulate the same volume. Regular aerobic exercise also improves the flexibility of your blood vessels and their ability to dilate, reducing resistance to flow.
Aerobic activity — brisk walking, jogging, cycling, swimming — can lower systolic pressure by roughly 4–8 mmHg on average, comparable to some single blood pressure medications. Aim for 150 minutes of moderate activity a week, spread over most days.
Resistance training twice a week adds further benefit and protects muscle and bone, particularly important with age.
The effect is not permanent. Stop training and blood pressure drifts back up within a few weeks — consistency matters more than intensity.
The other levers
- Salt. Reducing intake to under 6 g a day can lower systolic pressure by several mmHg. Most dietary salt is hidden in bread, sauces, processed meats and ready meals.
- Potassium. Vegetables, fruit, beans and potatoes help counterbalance sodium. The DASH eating pattern is built around this principle.
- Weight. Every kilogram lost tends to reduce systolic pressure by around 1 mmHg.
- Alcohol. Keep within 14 units a week; cutting back has a measurable effect.
- Smoking. Each cigarette causes a temporary spike, and long-term smoking stiffens arteries permanently.
- Sleep and stress. Poor sleep and untreated sleep apnoea both raise blood pressure independently.
- Caffeine. Causes a short-term rise in some people; worth noting if you measure at home.
Measuring at home properly
Use a validated upper-arm monitor. Sit quietly for five minutes first, feet flat, back supported, arm at heart height. Take two readings a minute apart, morning and evening, and record them. A single high reading means little — the trend is what counts.
A note of caution
Blood pressure rises temporarily during exercise, which is normal and beneficial in the long run. But if your blood pressure is already high, speak to your GP before starting a vigorous programme. Seek urgent medical attention for a reading above 180/120 accompanied by chest pain, breathlessness, severe headache, visual changes or weakness.
