Losing a significant amount of weight is one of the most meaningful things you can do for your long-term health. If you've achieved that with the help of Mounjaro or Wegovy, that achievement deserves to be recognised - not undermined by something nobody warned you might happen.

Loose skin after substantial weight loss is real, common, and something that catches a lot of people off guard. It can feel deflating when you've worked hard, lost the weight, and then find yourself frustrated by how parts of your body look. The feelings that come with it - self-consciousness, disappointment, wondering whether it was worth it - are entirely valid.

The same loose skin would occur after losing 5–6 stone through any means: surgery, diet alone, or exercise. The difference with GLP-1 and dual-agonist medications like Mounjaro and Wegovy is that they can support faster, more substantial weight loss than many people have previously achieved - which in turn makes the loose skin outcome more likely and sometimes more pronounced.

To put it plainly: the medication worked. The loose skin is a side effect of success, not of the drug.

Why Does Loose Skin Happen After Significant Weight Loss?

Your skin is an extraordinarily elastic organ. It stretches as you gain weight - accommodating the expanding fat layer beneath it - and ideally contracts when that fat is lost. But skin's ability to contract depends on a protein framework made up primarily of collagen and elastin.

When skin has been stretched significantly over a long period, those collagen and elastin fibres can become damaged, loosened, or less resilient. Once the underlying fat is rapidly reduced - as often happens during medically-supported weight loss - the skin may not be able to retract at the same pace, leaving excess skin behind.

Think of it like a balloon. A balloon that's been inflated for years won't snap back to its original size and shape the moment you let the air out. The longer it's been stretched, and the more it was stretched, the more slack it retains.

Which Areas Are Most Affected?

Loose skin after weight loss with medications like Mounjaro and Wegovy tends to appear most noticeably in:

  • The abdomen - the most commonly reported area, particularly after significant weight loss or following pregnancy

  • Upper arms - loose, hanging skin is common here and can cause frustration with clothing choices

  • Inner thighs - skin that hangs over or around the knee area is frequently mentioned

  • Breasts - volume and shape can change significantly with fat loss

  • The face, jawline, and neck - facial fat loss can lead to a gaunt or hollowed appearance, loose jowls, or crepe-like skin on the neck

  • Back and flanks - sometimes called "back rolls" or "bra bulge" - excess skin here doesn't always disappear with fat loss

The areas affected, and how severely, depend on where you carried weight and for how long.

Factors That Affect How Much Loose Skin You Get

Not everyone who loses weight with Mounjaro or Wegovy will experience significant loose skin. These are the main factors that determine the extent of it:

How much weight you lost. The greater the total weight loss, the higher the likelihood of visible loose skin. Losing 3–4 stone is very different from losing 8–10 stone in terms of skin response.

How quickly you lost it. Rapid weight loss gives skin less time to gradually adapt. The faster the fat disappears from beneath the skin, the more likely it is to lag behind. This is one of the most important factors.

How long you were overweight. Skin that's been stretched for a decade has had longer to lose its elasticity than skin stretched for two years.

Your age. Collagen and elastin production naturally declines with age. Younger people generally have more skin elasticity and a better chance of natural retraction. This doesn't mean older people can't improve their skin - it just sets realistic expectations.

Genetics. Skin elasticity has a strong genetic component. Some people's skin bounces back remarkably well; others' doesn't, even under identical circumstances.

Sun damage and smoking history. Both degrade collagen over time, reducing the skin's capacity to retract naturally.

Your hydration and nutrition during weight loss. Protein, hydration, and micronutrients all support skin health during the process.

"Mounjaro Face" and "Wegovy Face" - What They Actually Are

You may have come across the terms "Mounjaro face," "Wegovy face," or "Ozempic face" on social media. These are not medical diagnoses - they're patient-driven shorthand for a real phenomenon: facial fat loss that can make the face appear gaunt, sunken around the cheeks, or older-looking during or after significant weight loss on GLP-1 medications.

The mechanism is the same as elsewhere on the body - fat volume loss that the overlying skin doesn't fully compensate for. In the face, this can manifest as:

  • Hollowed or sunken cheeks

  • More prominent nasolabial folds (the lines from nose to mouth)

  • Looser skin around the jawline or jowls

  • A crepey or thinner texture on the neck

It's worth noting that weight loss itself - by any method - produces these changes in people who lose significant amounts of facial fat. The medications are associated with it because they enable substantial weight loss, not because they specifically target or deplete facial structures.

For some people, this is an unwanted side effect of transformation. For others, it's barely noticeable or not a concern. Individual experience varies considerably.

1. Lose weight steadily where possible

The rate of weight loss is one of the strongest predictors of loose skin. Realistic Expectations - and Why Loose Skin Doesn't Cancel Out Your Success

This is something worth saying directly.

Losing 5, 6, 8, or 10 stone changes your health in ways that extend far beyond how your skin looks. It reduces the risk of type 2 diabetes, heart disease, sleep apnoea, joint damage, and certain cancers. It can add years to your life and decades to your quality of life. For many people, it also transforms how they move, how they sleep, how they socialise, and how they feel about themselves at a fundamental level.

Loose skin is a real and sometimes emotionally difficult part of that transformation. It deserves to be taken seriously, and it deserves practical support - not dismissal. But it does not erase what you've achieved.
